The Centers for Medicare and Medicaid Services (CMS) has launched the Independent Dispute Resolution (IDR) Gateway, a centralized platform for managing disputes related to surprise out-of-network medical bills. This new system replaces legacy web forms and aims to streamline the dispute process for health care providers and payers. Starting November 13, 2026, users can submit various notices through the IDR Gateway, including the Notice of IDR Initiation and Notice of Offer. The platform has been developed in response to a significant backlog of disputes, with over 7.7 million disputes initiated since April 2022. Users must create accounts and verify their identities to access the platform, which includes dashboards for tracking disputes. The initial rollout will not support API access to dispute data or features for the negotiation period before initiating IDR. The transition to the IDR Gateway is expected to enhance clarity and organization in managing disputes.
