The breach occurred between June 5 and July 24, 2026, when an unauthorized party accessed a clinical user account within the WellSky electronic medical record system using compromised credentials. The breach was discovered on July 21, 2026, and was confirmed to be isolated to the WellSky platform. Information potentially exposed includes names, addresses, Social Security numbers, scheduling, and clinical information. The incident was reported to the Texas Attorney General's Office on September 21, 2026. Although no confirmed misuse of the data has been reported, the company is offering affected patients credit monitoring services. A class action lawsuit is being considered by attorneys for those affected.
