Doctor's Choice Care & Hospice, a health and hospice provider in East Texas, has reported a data breach involving personal and health information.

According to a notice posted to the provider's website , an unauthorized party accessed a clinical user account within the electronic medical record system provided by WellSky, a healthcare technology and software solution company that Doctor's Choice Care & Hospice utilized. This unauthorized access to the WellSky account occurred between June 5 and July 24, 2026 via compromised credentials.

Doctor's Choice Care & Hospice identified the breach on July 21, 2026. The company, after investigating with WellSky, confirmed that this account was the only point of unauthorized access and that the incident was isolated to the WellSky platform.

Information that may have been impacted during the incident included demographic information (such as names, addresses and dates of birth), Social Security numbers, scheduling information and clinical/treatment information.

The Doctor's Choice Care & Hospice data breach was also reported to the Texas Attorney General's Office on September 21. The report noted that the breach affected 14,333 Texans and may have also exposed health insurance information.
